Backup Data Restore Request

Complete this request if you are currently registered for Backup Support and require restoration of data that has been deleted, lost or corrupted on an actively backed up system drive, volume or partition.

Please note that the system and relevant drive, volume or partition must have been registered and actively backed up for restoration to be possible.

Backup Source Host: Hostname or CMU Asset Tag of the machine the file(s) were located on.
Backup Source Path: The full path and filenames of the requested file(s).
Please make sure that these are the actual paths of the file(s) and not a symlink/shortcut to them.
Backup Source Dates: The last known dates during which the file(s) existed.
If looking for a restore from a different date please provide the date you would like the restore from. Backup Source Dates: The last known dates during which the file(s) existed.

Restore Target Host: Hostname if there is an alternate host for the restore (if applicable).
